Eligibility
- Full-time employees (30+ hrs./wk.): Eligible upon hire; must choose benefits within 30 days of hire date.
- Eligible dependents: Includes employee’s spouse/domestic partner, children and legal dependents to age 26, plus disabled dependent children of any age who meet plan criteria.
